Prologue - Avengers (1st series) #115 [First Story] - The Avengers come to Black Knight’s castle to find him, but encounter a magical barrier and some subterranean bad guys instead. They discover that Dr. Strange is behind the barrier, and decide to seek him out.
Prologue - Avengers (1st series) #115 [Second Story] - By promising to restore his eyesight, Dormammu enlists a blind Loki in a plan to manipulate the Defenders into recovering the six pieces of the powerful Evil Eye.
Part 1 - Defenders (1st series) #8 [Second Story] - Dr. Strange contacts the essence of the Black Knight, who has been turned to stone. Dormammu and Loki intercept the message and respond as the Knight, prompting the Defenders to seek out the pieces of the Evil Eye as a possible cure to his condition.
Part 2 - Avengers (1st series) #116 [First Story] - The Defenders discover the locations of the pieces of the Evil Eye and head out to get them. Meanwhile Loki realizes Dormammu is a greater threat than he had thought, so he magically contacts the Avengers and convinces them that the Defenders are villains and must be stopped.

Part 4 - Defenders (1st series) #9 [First Story] - The Silver Surfer returns and notifies Dr. Strange of his battle, and Strange deduces Loki’s involvement. Meanwhile Vision contacts the other Avengers and tells them the Defenders have truly gone evil.

Part 10 - Defenders (1st series) #10 [Second Story] - Flashback to the Avengers and Defenders realizing they had been duped, thanks to Namor and Captain America. In the present, Strange reunites the pieces of the Evil Eye just in time for Dormammu’s servant to steal them, and begin corrupting the Earth with Dormammu’s magic.
Part 11 - Avengers (1st series) #118 - The two teams enter Dormammu’s dark dimension to battle him, but they are overwhelmed until Loki betrays Dormammu. The Scarlet Witch causes the Evil Eye to absorb Dormammu (who is made of magical energy) and blasts that energy at Loki, driving him mad.
Epilogue - Defenders (1st series) #11 - The Black Knight is restored.
Related Issues:
Defenders (1st series) #4 - Black Knight aids the Defenders against the Enchantress and is turned to stone. Dr. Strange erects a barrier around the Knight’s castle and takes his petrified body back to his manor in hopes of curing him.
Notes:
The Evil Eye was first seen in Fantastic Four (1st series) #54, where it was seemingly destroyed. In this story Dormammu reveals it was merely broken into pieces, which were scattered across the globe.
Loki was blinded in Thor (1st series) #207. The blast of energy from the Evil Eye, though it drove him temporarily mad, restored his eyesight.
Hawkeye joined the Defenders in Defenders (1st series) #7, during a conflict with Attuma and the Red Ghost. The second half of this story is the first story in Defenders (1st series) #8.
